Does any one know where or how I can get a replacement Form ACA-309 dated [12-45]? This is the Placard that must displayed in view of the pilot Mine is getting to be very brittle and unusable.

Maybe this is too simplistic, but can't you just make a copy of the one you have?

The form may be on the CD showing the AC history which can be obtained
from the FAA.

It is on the CD.... We have not had blanks here for many years!
